- Queer in Nigeria: Mental health struggles
Trigger warning: this article contains mentions of violence. Being an LGBTQ+ Nigerian, one of the most discriminated identities in the world, comes with a litany of mental health challenges. Many Nigerians suffer from mental health issues, but these are dismissed as a Western “woke” performance. Living as a gay Nigerian has made my mental health struggles pronounced and impossible to ignore. Mental health deterioration doesn’t suddenly manifest, but rather, it eats away at us in subtle, insidious ways. As a Nigerian gay man, I have struggled with my mental health for most of my life, as it has intertwined with other parts of my existence. My journey to accepting myself has been long and tumultuous. At around five, I was sexually harassed by my older female cousin, and years later, I further explored heterosexuality with a female friend. At age eleven, I discovered I was gay. I knew something about me was different, but due to a lack of sex education, I was left to grapple with confusing and unexplained emotional and sexual attractions. I couldn’t understand who I was. At fourteen, I finally accepted my sexuality, but this was not easy, especially given that I was navigating through various stages of religious belief and disbelief. At nineteen, I fully explored my sexuality. Throughout this journey to acceptance, I have faced a multitude of difficulties. Accepting my sexuality From the age of two, after the death of my mother, I lived with my grandmother (my father’s whereabouts remain unknown) until I was twelve years old, when I moved in with my uncle. While my family has become a part of who I am, discovering my sexuality has made me feel that I don’t belong with them. As I get older, living where I don’t feel I belong, both within my family and my country, makes me increasingly sad. With each passing year, I watch my dreams tied to a traditional idea of a heteronormative ‘family’ slip away. I had a rough academic journey at university. I couldn’t fully explore university life, as I was saddled with depression and anxiety. Yet, this troubled journey shaped many of my current beliefs. I wondered, ‘What can I do better to make things better?’ ‘How will I contribute to the fight for our rights?’ I knew that I would leverage the power of storytelling and reporting to contribute in my own way. LGBTQ+ rights in Nigeria workplaces In Nigeria, most workplaces are fixated on heteronormative family units. Thus, only two types of people exist: those who are and those who will be married. As a gay man, I cannot possibly fit in. Despite the stress, anxiety, and depression I must bear, I crave to live a normal life. However, I feel that I could never live authentically while working in a Nigerian workspace, where I will never be accepted or respected. I hope to one day set up my non-governmental organisation (NGO) and help advance LGBTQ+ rights in Nigeria, where there are just a few activists and organizations fighting for inclusive policies. For now, I have chosen to work as a freelancer and report on LGBTQ+ experiences in Nigeria. Sometimes, I feel like my whole life is failing me as a freelancer, especially when my contracts end. I often wonder if I should enter the Nigerian corporate world and make a stable living. Then another freelance contract comes in and funds two months’ earnings, and it is a godsend. I want to live authentically, and for now, this is the path I must follow to do so. Fears of violence This lack of belonging is coupled with a pervasive and constant fear of violence. On three occasions, I have escaped being set up on dating apps by heterosexual men pretending to be queer. In Nigeria, this is a common occurrence, and these people, pretending to be gay, have even earned their own slang name: “Kito”. The motives of Kito’s may vary – normally, they aim to kill, ‘out’ (expose a person’s sexual orientation/ gender identity without their approval) or extort their victims. Kito’s build a relationship with them online, then make plans to go on a date, at which point they will most likely cause physical harm and initiate blackmail of revealing their victims queer identity. It is only by following my instincts and investigating people’s details prior to meeting them that I have avoided certain violence. I also use kitodiaries at times – an online directory warning people of known Kitos. Unfortunately, others are not so lucky, and many gay people who have been set up bear the trauma of this abuse for the rest of their lives. Despite my narrow escapes, I am still filled with anxiety and worry whenever I meet someone new, as I can never be certain of their true intentions or my safety. These cruel ‘set-ups’ go unpunished due to Nigeria’s sexual offence laws, whereby homosexuality is considered a criminal offence, carrying a sentence of up to 14 years in prison if found guilty. Thus, if the victims of these violations come forward, they risk imprisonment. Some days, I sit and worry about what will become of me if I never leave the country. Feelings of isolation eat deep inside me. In these moments, I lie down and cry my eyes out. I want a future that has better legal rights for LGBTQ+ Nigerians.
- Song in the City: Taking Classical Music out of its comfort zone
I have been a trustee of 'Song in the City' for nearly 10 years now. I was initially drawn to their series of lunchtime concerts in the City of London. After that, I became interested in their socially engaged projects, which matched my work at Arts Council England. Song in the City is an artist-led registered charity, directed by Gavin Roberts & Rebecca Cohen, that takes classical music out of its comfort zone. We devise innovative interdisciplinary performance projects that connect artists, communities, organisations, and audiences Through the seemingly traditional genre of song, we have been working to revolutionise the relationship between classical musicians and their audiences through imaginative concerts, and social projects. Partnerships have been key to this, and in recent years have included Patrick McDowell for London Fashion Week, Guildhall School, Rambert Dance Company, MIND in Tower Hamlets and Newham, and the National Portrait Gallery. Healthy Lunch Breaks We go back to January 2011, to St Botolph’s Church Hall near Liverpool Street station where Song in the City started its free series of concerts called “Healthy Lunch Breaks”. These performances were designed around socially engaging themes allowing a connection with new audiences that might not otherwise attend a song concert. Thus, this series comprised concerts on the themes of love, Shakespeare in song, LGBT+, war songs, Black History Month, and others. And so, it kicked off what would become over 150 free lunchtime concerts, concluding in 2017. Office workers, local residents, music lovers, and tourists alike were all able to enjoy an hour of free classical song every week. Artistic directors and Professorial staff at the Guildhall School Gavin Roberts and Rebecca Cohen curated and performed at these concerts, drawing on student performers from the Guildhall School. The artistic goal was always to find new ways of presenting classical songs beyond the traditional recital format. Theatricality, drama, and movement all became touchstones of these performances. “I initially wanted to study piano accompaniment (at the Guildhall) because I love music, but I've come to realise that the world it inhabits can be stuffy and intimidating. That should never be the case - we grow up singing songs and I hope Song in the City captures the joy in that." - Gavin Roberts, Artistic Director, Song in the City Creative Minds in Song Parallel to the “Healthy Lunch Breaks”, Song in the City was developing a focus on how music and song could positively engage people with mental health challenges and those in the mental health services. In 2013, we started our flagship project, now called 'Creative Minds in Song'. Creative Minds in Song brings mental health service users with lived experiences of severe mental illness as poets, together with composers, singers, and pianists from the Guildhall School and previously King's College London. They work together to transform the poets’ (patients’) words into a recognisable poetry format by collaborating with writers and then turning this poetry into music and song through a series of facilitated workshops. At the heart of the project is the simple idea of finding a way for the songs to reflect the poets’ experiences, knowledge, and understanding of the mental health system through their personal experiences, and what it is like to live in a society that continuously stigmatises people living with mental health challenges. The songs tell the stories of these experiences and are performed in front of an audience, resulting in poignant, emotionally true representations of the experiences providing direct insight into what it is like to live daily with mental health challenges. Creative Madness in Song 2017 There’s substantial evidence of the transformative nature of music for people with a wide range of mental health challenges, such as schizophrenia and depression. Here, the format of traditional classical music recital breaks out of its clichéd “white tie and tails” stereotype and becomes something owned by the poets themselves, some of whom also perform reflecting their own participation, characters, and needs. Pairing trained musicians with people who have real stories to tell can produce a result like none other. While the poets themselves gain much from the project, the project can have a transformative impact on the musicians and artists, often encouraging them to confront their own feelings and emotions as they work to transform the poets' emotionally rich experiences. At the most basic level, the musicians and singers themselves have the opportunity to engage in an environment often far removed from the starchy world of classical music and the view that most people have of it. The process of the project and how participation happens is paramount, with the workshop format being the heart of it, and each voice being equally valued as part of the development of the songs. Workshops provide an environment for mutual encounter and learning, and occasions to celebrate the group’s creative achievements before sharing them in performances. Contact and connection between mental health service users, poets, composers, singers (training professionals and mental health service users themselves), and pianists is crucial at all stages of the process so that a group mentality is adopted to enable the best sharing of creativity. This aligns with the 5 evidence-based actions to improve personal wellbeing (Connect, Be active, Take notice, Keep learning, Give) of The New Economics Foundation. Song in the City is currently planning another iteration of the Creative Minds in Song project and we are in the process of looking for partners and of course, funding, to make it happen. This is a so much needed initiative as the project breaks down stigmas associated with mental health issues creating a link between mental illness and creativity. “After all the years of fighting and survival, my life is opening up again through the wonderful creative minds in the song project and my wonderful composer. - How I’ve overcome queer shame by helping others
Growing up LGBTQ+, I always felt different, like I didn’t belong. Constantly feeling like I didn't fit in, a lack of representation and the use of casual homophobic slang was extremely detrimental to my mental health, leaving me feeling depressed and anxious. However, last year I began working for an LGBTQ+ health and wellbeing charity and now I am helping others with their mental health while overcoming the shame I have for my own identity. I distinctly remember the first time I heard the word ‘lesbian’ shouted across the playground as an insult. I was probably only around 8 or 9 and definitely wasn’t sure of my queer identity yet. However, I remember how, while not even directed at me, it hit me like a tonne of bricks. I believe this moment was a catalyst in the long and complicated journey I have had with my sexuality. Coming to terms with my sexuality was an extremely lonely and isolating time in my life. It’s a bit of a cliché, however, the gut-wrenching feeling of being sat around with a group of girls discussing their crushes on boys, knowing that you did not relate, is an experience I am all-too familiar with. Simultaneously, LGBTQ+ representation in 2000’s -2010’s pop culture was rare, but when we did see a queer woman in TV or film, they were either over- sexualised or seen as a ‘joke’, a negative stereotype that I very quickly internalised. This led to a huge feeling of shame, making me feel worthless and like I wasn’t good enough. Despite having good friends in my early teens, as secondary school progressed, the shame only grew, which led me to push myself away from those close to me and become extremely lonely. I know that I’m not alone in this; A study by Just Like Us found that young LGBTQ+ people are twice as likely to suffer with depression, anxiety and panic attacks than their non-LGBT peers. Not only this, but they are also twice as likely to contemplate suicide than their non-LGBTQ+ peers. Cut to several years later, unfortunately, I find that this sense of shame that I felt so strongly growing up, is still with me to this day. I will often still find it hard to ‘come out’ to people and it is my default to feel like I am not ‘good enough’, whether that is in friendships, relationships, work or even hobbies. However, I have come an awfully long way since I was that scared and lonely teen. In particular, university gave me the opportunity to meet a huge range of new people, as my authentic queer self and with no preconceptions of who I used to be. I now feel incredibly lucky that I am surrounded by many other beautifully queer people and supportive allies. While my mental health has been significantly impacted by my LGBTQ+ identity, it would be ignorant not to acknowledge the fact that we have seen significant improvements in attitudes towards LGBTQ+ people over the last 50 years. This, of course, is due to the numerous LGBTQ+ organisations and campaigners who have been supporting the community and fighting tirelessly for the rights of LGBTQ+ people. London Friend, who I now work for, being one of them. For me, working for London Friend is a way of giving back to all of those courageous queer people who have come before me and paved the way for generations of queer people to come. Similarly, I know that my experiences are also made better by the fact I am white, cisgender and have an incredibly open minded and supportive family. On the other hand, LGBTQ+ people of colour are more likely to experience homophobia or transphobia than white LGBTQ+ people. Acknowledging my privilege was a huge motivator in wanting to help other LGBTQ+ people. This is particularly poignant at a time where transgender identities are being vilified in the media, in what is being called a ‘culture war’ over trans rights. I cannot begin to imagine the distress it must cause to constantly be hearing your own identity, something so integral and personal, being debated and discussed. Therefore, it would be a disservice to my younger self, who felt so lonely and ashamed of her identity, not to do what I can to support those who are experiencing the right to live as their true selves being questioned. As the oldest LGBTQ+ charity in the UK, London Friend have adapted their services over the years to serve the needs of the community. Right now, they provide three main services; social support groups, 1-2-1 counselling and drug and alcohol support. All of these services address the unique needs of LGBTQ+ people to support their mental health and wellbeing. Throughout my time at London Friend, I have witnessed how these services can have a positive impact on people’s mental health. From seeing a drug and alcohol client celebrate a sobriety milestone, to a member of our over 50s group connecting with new LGBTQ+ friends, it is undeniable how both the practical support and general sense of community at London Friend can be a vital tool in supporting LGBTQ+ people and their mental health. Of course, there are some tough aspects of the job. Hearing about people’s poor mental health and the challenges that they have faced can be incredibly upsetting. It is often hard not to take those stories home with you. In particular, when I was new to the role, it was certainly quite taxing on my own mental health as many stories would bring up past memories of my own. However, the solidarity and support from the team is second to none and I am constantly inspired by my colleagues' expertise and professionalism, which reassures me that by accessing our services, those struggling with their mental health are in the right place. Certainly, one of the most rewarding aspects of my job is reading clients feedback as it is almost always so positive, enthusiastic and emotive. Each time a client recalls how something about our service was able to help them make change in their life, I can feel myself dismantling the shame I felt for so long, making me feel like the struggles I went through were worth it to get to where I am now. From feeling like an outsider, to becoming a member of a rich, supportive community, I feel so incredibly lucky to be part of London Friend and I am incredibly proud of myself for getting where I am today. While, of course, not everyone should or will go to the extent of using their past experiences as the basis of their career, I cannot stress the value in finding your community and looking out for others. It might just help you on your own personal journey.
- Understanding mental health in LGBTQ+ people
Research on biological and psychosocial factors in mental health disparities offers clues to reducing the mental health burden among LGBTQ+ people. In Pride month, one of the areas that will be much discussed is mental health among LGBTQ+ people. My name is Qazi, and I’m a scientist and lecturer at King’s College London. I’ve been studying the origins of sexuality for about 20 years, and I am now working on understanding the factors involved in mental health problems among LGBQT+ people. This field is fascinating for its own sake because sexuality is central to human nature and our deepest passions. But this research can also help improve the lives of LGBTQ+ people. Here I describe some of the work that myself and others have been doing. But let me start with a word about terminology. In our community, we have a bit of an alphabet soup of different acronyms we use. In this article, I’ve used “LGB” to stand for lesbian, gay, bisexual, and in other places “non-heterosexual” where the studies use broader groups. I’ve also used “SGM” or “sexual or gender minority” which is a more recently coined broader term for people who are LGBTQ as well as LGBT+. This includes people who may not identify as such but include those who have same-sex attractions or engage in sexual relationships with people of the same sex or gender. People who identify as lesbian, gay, and bisexual (LGB) are at almost twice the increased risk of common mental health problems, such as depression, anxiety, suicidal thinking, self-harm and substance dependence compared to heterosexuals. Bisexual people also appear to have even poorer mental health while lesbian and bisexual women have a particular risk of substance dependence. So LGB people have an increased need for mental health interventions, but we know very little about whether they benefit equally compared to heterosexual people from the mental health interventions currently on offer. My colleague Professor Kate Rimes investigated this by analysing data from all the NHS Talking Therapies programmes in England (this is a programme that provides psychological therapies for common mental health problems on the NHS). What she found was that for psychological interventions offered for depression and anxiety, lesbian and bisexual women had poorer treatment outcomes than heterosexual women. Bisexual men also had poorer treatment outcomes than heterosexual and gay men. So, we need to improve treatments for these groups. One thing that would help us to improve our treatments is if we had a better understanding of the causes of mental health problems in LGBTQ+ people. Some researchers argue that so-called “minority stressors” cause mental health problems in LGB people. These stressors include the discrimination and stigma they experience (such as homophobic abuse) as well as psychological factors such as hiding one’s sexuality or gender identity, and their own negative beliefs about being LGBTQ+ (sometimes called internalised homophobia). These different stressors may interact with each other to cause mental health problems. While these factors are enormously important, we also wanted to test whether genetics played a role. Are LGBTQ+ people genetically more vulnerable to experiencing mental health problems? In one study, we did with our PhD student Liadh Timmins, we recruited identical twin pairs (who share all of their genes) where one twin was heterosexual and their co-twin non-heterosexual. We then asked them to complete surveys about their mental health and experiences of minority stressors. Since the twins already share the same genotype, genetics cannot be the explanation for any differences found. We found that there were no differences between the twin pairs in depression, anxiety, and well-being, although the LGB twins had higher rumination than their heterosexual co-twins. Rumination is a vulnerability factor for depression and anxiety and involves overthinking or trying to think about why you are feeling low. Our study has two main implications: yes, there is some influence of genetics in their experience of mental health problems, as both twin pairs had similar levels of depression and anxiety, but also that social experiences unique to each twin influence mental health, as only the LGB twins had increased rumination. This suggests that targeting rumination might be one avenue for researchers and clinicians to consider in improving mental health among LGB people. At this point in our understanding of the science, we have some evidence that genetics influence some mental health problems in non-heterosexual people, but not necessarily on the specific psychological factors (such as rumination) that we might be able to change. The next step is whether we can identify additional modifiable psychological factors that could be potential targets in future psychological interventions. We tried to do this in two studies where we followed up people over long periods of time. This method helps us to better understand whether one factor causes another over time. In the first, Dr Kunle Oginni found that people who identified as a sexual minority at age 15 went on to report more suicidal ideation and self-harm at 20 years of age. Importantly, he also found that low self-esteem measured at 17 years was an important factor in this link. This shows that targeting self-esteem among LGB people might help us to reduce suicidal ideation and self-harm later on, as well as depression. In a second study, our PhD student Georgina Gnan studied the mental health of LGBT university students over three time points, a month apart. She found that having lower self-esteem and more negative beliefs at earlier time points was associated with more depression, anxiety, and suicidality later on, because of more rumination and also more avoidance (such as avoiding difficult situations) and hiding one’s LGBT identity. My colleague Professor Kate Rimes is drawing on these and other findings from our research to develop a new intervention to target low self-esteem in young LGBT+ adults, and the early results appear promising. In concluding this piece, I think it is important to note that many factors influence mental health disparities between LGB and heterosexual people. Discrimination and stigma are important but do not entirely explain these differences. Genetics does not explain it away completely either. Thus, there are many more factors at play here. It is also important to end on a positive note and I encourage readers to remember that based on our scientific knowledge about the rates of mental health problems in LGBTQ+ populations, most people who are LGBTQ+ will not develop mental health problems, and of those who develop them, few are attributable to being LGBTQ+. And most importantly, new psychological interventions developed specifically for this community are coming soon.
- Has the $1.8 trillion Wellness Industry commodified mental wellbeing?
Commodified = to treat or consider something as a commodity (= a product can that be bought or sold) - Cambridge Dictionary More than ever, we are prioritising our health. Rooted in this desire for optimisation, an economy has flourished, and relatively quickly at that. Wellness has become a trillion-dollar industry offering a range of products and services on a constant conveyor belt; from biometric rings, vitamins, supplements and juices, infrared saunas, mindfulness apps, and aesthetic products, wellness products come in all shapes and sizes. And that’s not even scratching the surface. A dedicated sector for mental wellness is one of the fastest growing, alongside our awareness of our mental health and wellbeing. Personally, I like to dip my toe into the wellness waters. It doesn’t take much for me to add immunity shots to my weekly food shop, or to pop an all-frills skincare product in my online basket. There’s something I find satisfying investing in the idea of a ‘better’, ‘healthier’ you – even if it does come at the expense of your bank balance. Sometimes you (or at least I) just can’t resist. ‘The wellness industry is selling you the myth that a healthy lifestyle is expensive’ reads a headline from the Guardian in 2020, proving that the industry is not without its critics. ‘Wellbeing is presented as complicated, complex, difficult to achieve correctly and best when purchased – all the while requiring gurus to access it’, the journalist pens. But yet, it doesn’t exist in a silo. If it weren’t for consumer interest, surely there would not be quite so much success. While personally, I don’t quite relate to the Guardian journalist’s ‘existential pain’ surrounding modern-day ‘wellbeing’, I completely understand the sentiment behind the statement – wellbeing has become somewhat of a product despite the concepts’ existence when you strip it away from the business lens. It begs the question, is the hunger for consumerism and company profit just tapping into the proverbial saying that ‘prevention is better than cure’, or is there really a role for the industry in our wellness? With a dedicated sector, do we have to buy our way to better mental wellbeing? What is mental wellness anyway? ‘Wellness’, often used interchangeably with ‘wellbeing’, is the state of being healthy, especially when actively trying to achieve it, moving towards optimisation. The concept of wellness is nowhere near as new as the industry, with the Global Wellness Industry perfectly describing that ‘wellness is a modern word with ancient roots’. And ancient roots indeed – the earliest records of the idea go as far back as Ayurveda in 3000-1500 B.C, a holistic system of medicine from India focussed on improving health or maintaining it. Traditional Chinese Medicine, ancient Greece and Rome also reflect these principles of working towards prevention rather than cure. Mental wellness itself doesn’t have one set definition but is generally about how we feel and the ability to cope with daily life. According to the Global Wellness Institute, where the industry places its role in mental wellness, is helping with ways of coping with everyday challenges such as stress, burnout, loneliness and sadness outside of clinical (psychological and psychiatric) care. Products and services in this sector appear to offer mindfulness guidance in the forms of apps, sleep supplements, tech for tracking mood, movement and sleep, supplements and aromatherapy-based products, to name a few. But is this needed? Well, the short answer is no, not really. Criticisms of the industry tend to lean on the fact that a healthy lifestyle can be basic and stripped back – healthy nutrition, daily movement, avoiding substances, and getting sufficient sleep. And this is valid. ‘Mind’, a prominent mental health charity, provides tips to help improve mental wellbeing including trying to relax and reduce stress, spending time in nature, looking after your physical health, and improving sleep. Most of these should, and I emphasise the word ‘should’ for a reason, be readily available to all, rather than a product to be purchased as a consumer. Unfortunately, there are still obstacles creating barriers to improving things such as physical health with billions of people being unable to afford to eat healthily, an alarming consideration beyond the realm of the Wellness Industry. That mental wellness is personal and subjective is acknowledged in the industry, but the products offered can’t necessarily account for the degree of personalisation involved in achieving an individual’s mental well-being and many factors, such as adequate nutrition, are far beyond its scope. Mental health hinges on several individual, social, and structural determinants varying throughout life and acting as either risk or protective factors. While some of the individual determinants such as emotional skills can be a target for change, others such as genetics cannot, and some of the more social and structural determinants such as poverty, inequality and environmental deprivation require systemic change – not a product to be purchased or trialled. While this isn’t a lesson or criticism of the industry (they certainly don’t try and sell a solution to all these determinants), it is a point to be made of the narrative surrounding it. An exclusionary wellness club? Labelling it an exclusionary wellness club may be a little extreme but, it’s important to realise that accessing the Wellness Industry, requires you to be in a relative position of privilege. It’s a point I wanted to make when I commented on my own privileged occasional splurge on an immunity shot or skincare product, but it seemed too early to mention. I recognise that I am fortunate to be able to do this, and unfortunately, this is not the case for everyone. It seems to me that the narrative is the most important point of consideration here: it’s important to avoid dictating that buying into mental wellness is essential as this isolates a huge amount of the population. The fundamentals of mental wellness are simple (in theory); anything you want to do beyond this is optional and fine so long as you have the choice. Mindfulness practices are a great example of this. The app ‘Headspace’ (not an ad) is probably one of the better-known options for such a service on the market. Claiming to be ‘the most science-backed meditation app’, they boast stats such as a 32% decrease in stress after 30 days and a 22% increase in focus following a single session. There’s certainly not an issue in opting for this kind of benefit if you can. However, there are also lots of free resources to help teach and guide mindfulness practices which are more freely available (although it is important to note that Headspace is free to download however, some content lies behind a paywall). In my opinion, mental wellness, and wellness in general, hasn’t necessarily been commodified, so long as the narrative doesn’t lead us to believe it is something we have to invest in financially. There’s no apparent large, evil, money-making scheme aiming to scam us all out of our hard-earned money. From a very naïve stance, there’s no profit without desire, and the Wellness Industry feeds that appetite with no ill intention. But making it clear that the products and services are somewhat of a luxury is essential. The risk, so clearly pointed out by the Guardian journalist, is that wellbeing is made to sound complicated and hard to achieve, and this should not be how it is portrayed. There’s certainly an argument (or a few) to be made about the narratives selling the products, and possibly even the scientific validity of some of what’s being rolled out. But with so much to say on the topic, perhaps that’s best saved for another article.
- American Fiction vs Non-Fiction
Exploring the conflicts portrayed within the award-winning film and what it reveals about America’s fraught relationship with race and perpetuation of media typecasting. Within the first five minutes of its run time, the movie American Fiction manages to seamlessly illustrate the double-edged sword of racial awareness. Monk, our sarcastic protagonist, explains that he doesn’t “even really believe in race” to which his agent replies, “Yeah, the problem is that everyone else does”. This short yet salient exchange captures one of the paradoxical issues that exists when it comes to race in America, as individuals adapt from being colour blind, they in turn become blinded by colour. American Fiction succeeds in unapologetically illustrating these tensions and proving that, when it comes to representation, America truly only goes skin deep. As a lover of the arts and a Nigerian American, I was very passionate about writing this article. Race is a tricky subject to broach within entertainment, however American Fiction finds a way to use satire in a way that strengthens its message rather than clouding it. Monk, a middle-aged Black man, is a writer and professor currently struggling in multiple areas of his personal and professional life. To make matters worse, he is constantly reminded of the success of first-time author, Sintara Golden’s novel We’s Lives in da Ghetto. Her book seems to offer little literary value besides entertaining tired stereotypes of Black individuals. Even so, her novel is positioned at the front of bookstores while Monk’s novels are erroneously placed in the back corner of the shop under African American history. Tired of being told his books aren’t “Black enough”, Monk assumes the fake identity of convict Stagg R. Leigh and creates an autobiography My Pafology (later renamed 'F#%*'). To his dismay, this faux autobiography is a hit and Monk must juggle confronting the problems of his real life with the success of the fantasy he created. Monk vs Sintara: The commodification of Black trauma During Monk and Sintara’s first direct conversation, we see Monk express his confusion over her acceptance of stereotypical tropes. Sintara contends that her work is intended to represent the real experiences of a pocket of Black individuals. Moreover, if the public wants this type of literature, then there’s nothing wrong with providing it. This exchange made me wonder why this isolated experience is what the public wants. Why is it that oftentimes Black people feel irrelevant in the media unless they have a struggle story to push? The problem isn’t giving such stories a platform, but rather the way in which they are packaged for the public. In such cases we are less interested in the message, than we are with pandering to the audience’s perception of Black stories. It is telling that the successful novels in the film that are treated as pinnacles of Black experience are somewhat inauthentic to the authors and purposely pander to publishers’ desire for Black trauma. Moreover, Monk’s past attempts to publish work true to his own ideas are disregarded and deemed as not Black enough…by White critics (please note that descriptions of White audiences are in context of the film). Because of this, authors like him are syphoned into creating work centred around broken families, incarceration, abuse, and poverty. While this is certainly an experience for some people and shouldn’t be invalidated, the fixation on trivialising these matters for a non-Black audience is problematic. And the reason it is done is because it sells. This creates stories of caricatures and tropes that Black people either can’t relate to or feel pressured to fit into. There seems to be two options for Black characters in literature and films, “the comedic relief friend” of the White protagonist (seen in movies like She's All That, Clueless, and Gone with the Wind) or “the victim” of society’s misfortune (seen in movies like Queen and Slim, For Colored Girls, and Them). While some recent films, such as Black Panther, subvert these stereotypes by showcasing and celebrating black heroes, the overwhelming majority of film and literature feature depictions of Black individuals that are one dimensional or exploitative. These narrow tropes are continuously shoved down our throats, yet the sellers of culture, like the publishers in American Fiction, believe that they have achieved proper representation. Furthermore, the persistent portrayal of Black caricatures can have damaging effects on the formation of identity through the phenomenon of double consciousness. Monk vs Stagg: A model of double consciousness The theory of double consciousness, created by W.E.B Du Bois refers to the fracturing of marginalised minorities, for the sake of assimilating into a society which rejects them. Another similar concept is “code switching” which is a more modern term that describes when individuals adopt language and mannerisms different from their own to fit into their given community. In a way, code switching is an external display of the internal identity conflict of double consciousness. Through the course of the film, we see Monk finally receive public praise for his work, the rub is that it isn’t him who is a success, but Stagg, a caricature created to satiate non-Black individuals’ concept of “Black” literature and experience. The paradox of Monk and Stagg’s separation within the context of one individual is a perfect visualisation of double consciousness. Although Monk never shows a desire to become Stagg, during various points we can see his outrage and disappointment that publishers and even people close to him are enamoured by the fictional character’s autobiography. Monk even ends his relationship with his girlfriend Coraline due to her approval of the novel (even though he wrote it). This is because although Stagg is a part of Monk, they are in competition. As humans, it is common to have several different aspects of self, however in the case of double consciousness the identity is less so of an integration of self, but a mask. Moreover, the more acceptance that Monk garners as Stagg, the more difficult it is for him to put down the mask. This is one of many reasons why popularising and promoting a skewed view of Black identity is extremely damaging. To avoid being typecast as an “Oreo” or too “ghetto”, many individuals of colour must adopt their own versions of “Stagg”. Although one could argue that you shouldn’t care what people think and be your true self, oftentimes the individuals who fall in the category of double consciousness lack the privilege to do so. American Fiction succeeds in showing some of the granular ways in which this occurs. As seen with Monk, failing to conform to stereotypes changes job outcome and in more extreme cases can be the reason why people are denied work, housing, and several important opportunities. America vs “race”: what now? American Fiction shows how we place implicit expectation on minorities to conform to narrow and ignorant views of identity. We need to move away from the pathological need to categorise everything into a neat box as we often do when we say: “the Black experience”. As Monk teaches us, his past stories are by definition “Black stories” not because they confine to someone’s prejudiced views but because he is Black, and he has written a story. Moreover, not everything is meant to be funnelled through a race lens. Once we truly internalise this concept is when we will broaden our perspectives as a society and stop invalidating the experiences of people of colour.
- A deep dive into the concept of influencing
Like many people in their 20s, I have a TikTok account (you can learn more about this social media platform in this article). While I don’t use it to create video content, I follow numerous creators (more popularly called "influencers") in the sphere of wellness, self-care, and travel. I find that watching their videos relaxes me, and motivates me to focus on my own wellbeing, to a certain extent. However, something that I’ve become more conscious about lately, is that influencers, true to their name, occasionally ‘influence’ me to buy products they recommend, like the latest skincare products, for example. A recent de-cluttering session made me aware of the products I’ve purchased "just because" I’ve seen my favourite creator talking about them, and I want to try them for myself. Some of the products I’ve been satisfied with, but the others, I’ve realised are indeed just a "hype", and not worth the money I’ve spent on them in an impulse purchase. This got me thinking about the science behind influencer marketing, which refers to collaborations between companies and content creators, to promote specific products or services. Who is an influencer, and how does influencer marketing work? Before we go ahead, you might be wondering who exactly an influencer is, and how they differ from celebrities. Influencers gain fame and traction within specific niches, such as fitness, beauty, or travel and therefore, have expertise within these sectors. Their followers are individuals who have a special interest in that topic. For example, if someone’s interest is travel, they’d follow a travel influencer’s journey around the world, who would occasionally endorse travel-related products. In comparison, celebrities are followed on social media because of an admiration for their talents in the entertainment industry, like musicians or actors. This form of marketing differs from products marketed and endorsed by celebrities, in that products are tailored to the influencer’s niche. Let’s take the example of a beauty influencer marketing a new facial moisturiser from Brand X. Brand X would pay the influencer a flat rate to feature their moisturiser in videos, with a link to purchase the product. This influencer would also provide a ‘promo code’; for each purchase made using the promo code, the influencer receives a certain amount in an affiliate commission. Personally, the reason why I have been more influenced to buy products used by content creators I follow, as compared to seeing a celebrity endorsement ad on the tube ride home, is the degree of familiarity with influencers. They, like us, are ‘normal’ people, most of whom also have day jobs, and use content creation as a hobby, and/or a means to earn extra income. They post daily videos, sharing snippets of their lives with their followers, which in my opinion, adds a layer of authenticity that celebrities often cannot provide. This is called the trans-parasocial relation. I appreciate the authenticity of the influencers that I follow because they do not shy away from admitting when they’re having moments of sadness or a lack of motivation, which is something that we all can relate to. Influencing for a good cause One of the many ways influencers can truly use their “influence” is by using their platform for a good cause and raising awareness about important issues, especially mental health. Some influencers share their own lived experiences with mental health conditions, while others are medical doctors, like Dr Jake Goodman, who share information based on their professional capacity. Another example of such an influencer is Dr Ally Jaffee (who recently wrote this article for ITM). Dr Jaffee is an NHS Junior Doctor specialising in psychiatry, with lived experience of mental health conditions, and uses her platforms for a good cause: increasing mental health literacy. Apart from mental health, one of the positive ways I witnessed influencers use their reach was during the second wave of the COVID-19 pandemic in India, when there was a tremendous shortage of healthcare resources like hospital beds, medication, and ventilators. Influencers from various niches came together and shared content highlighting the availability of resources and contacts in cities across the country, for their followers. However, with these positive features, it's important to always check for the credibility of information. While numerous influencers like Dr Jaffee and Dr Goodman are credible sources of information, many creators share content that’s inaccurate and potentially harmful. So, before you click the 'Follow' button on social media, consider the authenticity of their accounts, and make sure the information you receive is verified. Influencing has a not-so-good side too As with virtually any topic related to social media, influencing also has two sides to the same coin. In addition to the benefits discussed above, influencers might sometimes promote an inauthentic lifestyle in a bid to get more followers. For example, in the fitness-influencing industry, posting overly thin, muscular bodies can negatively impact the mental health and body image of their followers. In fact, research has shown that women who are exposed to the ‘thin ideal’ show higher body and facial dissatisfaction. One of the glaring negative aspects of influencer marketing is endorsing products that might be potentially harmful, such as weight loss pills, or unsafe skincare or makeup products. Additionally, while most influencers these days disclose when their social media post is part of a paid advertisement, some might not specify that their post is sponsored. This means that their content is misleading, and their recommendation might not necessarily be authentic. Aside from the consumer perspective, the influencer perspective also needs to be considered when evaluating the good and bad sides. The constant need to stay relevant in social media can cause immense pressure on content creators, which can, in turn, affect their mental wellbeing. Fighting to maintain relevancy in their niche can add a layer of competition with similar influencers, which would be another factor impacting their mental health. In response to the question “What do you want to be when you grow up?”, today’s children say they want to be “Youtubers” (or social media creators) as compared to the oft-responded occupation of an astronaut. It's clear that content creation and influencing are here to stay, so it’s important to know the realistic pros and cons that come with the jobs, to ensure that the mental health of our future generation is protected.
- Birth Trauma and Mental Health - the 'Postcode Lottery'
As a researcher in perinatal psychiatry (working with women who are pregnant or have recently had a baby), unfortunately I was not surprised to see last month's headlines about the Birth Trauma Inquiry report and the ‘Postcode Lottery on Perinatal Care’. I work in London, but even within boroughs across the city, women tell me of vastly different experiences of perinatal care. I also find that in the majority of moments when I tell people what I do for a living, they are quick to share stories of either their own experience, or the experience of their loved ones. One thing is clear to me – you don’t have to look very far to find a woman who has had a poor perinatal experience. Birth trauma is complicated, but what we do know is that it can have a profound impact on the woman herself, her family, and the early weeks and months of an infant’s life. Of course, birth trauma is very often coupled with significant physical pain and recovery, which is not to be ignored, but given that I am a researcher in perinatal mental health, this is the aspect on which I will focus here. According to the Birth Trauma Association, 4-5% women develop post-traumatic stress disorder (PTSD) after giving birth (as well as 1% of birth partners, which is also significant). This is important, because as well as the symptoms being very distressing, postnatal PTSD can also affect parents in other ways, such as how well they feel able to bond with their new baby, as well as how anxious they feel about having another baby in the future. Not only that, but the relationship babies form with their parents can impact their outcomes throughout life, as has been written in this article by my colleague Alessandra, so birth experiences are important for the whole family. As an example, Louise Thompson (who formerly starred in reality show ‘Made in Chelsea’), recently shared that her poor mental health after her traumatic birth experience impacted her initial connection with her son, saying that “when I did see him, I would hold him at arm’s length. I didn’t want to talk or sing to him.” The report contains the voices of many other women who have experienced birth trauma, but given how upsetting their stories are to read, I will leave readers to decide whether they feel comfortable seeking them out. Postnatal PTSD can be caused not just by physical trauma, such as difficult or painful labour and delivery, but also by psychological factors, such as feeling a lack of control in their delivery experience, or anger at medical staff who may not have cared for them or listened to them as they would have hoped. Indeed, in stories I have heard from women that I work with, it is clear to me that these psychological factors and a person’s perception of an experience play a large role in their risk for trauma and PTSD. For example, I have met women who, to those around them, would seem to have had a very physically traumatic birth, but who felt in control and cared for, and they suffered fewer symptoms of trauma and PTSD than other women who had an objectively less traumatic birth physically, but felt scared and unheard throughout. This importance of perception gives me hope that we can truly make experiences much better for women. Even in the best of facilities, with the best staff and circumstances, unfortunately there will still likely be a small minority of women who have unavoidable complications of childbirth which result in physically very traumatic deliveries and post-birth care. However, we have far more power to influence how a woman feels about, and remembers her delivery, based on the level of care that she is provided, both during the labour and delivery itself by the obstetric and midwifery staff, but also by psychology professionals after the experience. A great example of this that I have heard a handful of times in my work, is the ‘Birth Reflections’ initiative, an opportunity for women to have a debrief a few weeks after their delivery, with the obstetric professionals who cared for them, possibly with the addition of a psychology professional. These meetings are spaces to reflect upon aspects of the delivery that the women felt to be traumatic, and for them to gain further information on the circumstances of certain treatment decisions that were made in a hurry, in order that they can regain some understanding of what happened to them, and a feeling of control of the situation. In many cases, this has made a real difference to the women to whom I have spoken, but unfortunately it does not seem to be offered in every hospital yet. Of course, it is worth mentioning that not every woman who gives birth will have a bad experience. I work with women who suffer with postnatal depression, and so it is likely that I hear a higher proportion of stories of poor birth experiences than exist within the general population of women who give birth, because we know that perceived birth trauma is a risk factor for postnatal depression. Nevertheless, birth trauma is something that everyone who works with perinatal women and their families should be working to minimise as much as possible, because when it does happen, the effects can be profound and long-term. I was upset to read in the Birth Trauma Inquiry report that “women in their 60s and 70s wrote about how the memory of the birth was still vivid”. It is important for me to say that I do not blame healthcare professionals for much of this, as I know how hard they work and how much they care for their patients, despite staff shortages and often stretched conditions. The report makes a number of recommendations to improve care for women with respect to birth and trauma, including the recruitment of more staff, ensuring continuity of care, the importance of asking mothers about their own physical and mental health after the birth (not just after the wellbeing of the baby), the rollout of the ‘Birth Reflections’ programme, and efforts to tackle inequalities in care among ethnic minority groups. While small steps in these directions are happening all the time, thanks to the hard work of NHS staff and others, it is imperative that the government takes notice of this report and commits effort and funding to this important area. Only then will we truly be able to improve the experience across the country of women and their families at such a significant time, and make it memorable for all the right reasons.

Allo spopolamento per ferie non crede più nessuno e invece ecco che si scompare, ci si volatilizza, ci si rintana, Non dobbiamo dimostrare niente a nessuno, di essere ricchi, di essere poveri, o semplicemente di esserci. Siamo in una strana età, non credi? Ma vuoi sapere una cosa? Sono di buon umore. Oggi si fa festa. Laura: È il mio sette agosto. Aida: Il sette agosto. Quando sei nata non si trovava un taxi, non c’era un negozio aperto, siamo entrati in clinica e non funzionava nemmeno l’aria condizionata. Un’avventura. Laura: Tempo fa ho trovato delle foto. Tu ed io abbiamo condiviso, da quando sei nata; questa casa grande è tutta tua; così ho voluto, per il tuo bene, gioia mia! Laura: E allora? Adesso lasciami stare. Aida: Ho avuto una giornata pesante: forse devo lasciar perdere il laboratorio. I foulard di seta non piacciono più. E chi li compra si accontenta di quelli cinesi. Troppe beghe, gli impiegati che si lamentano, perfino gli stagisti. I clienti che non pagano… E tasse. Troppe tasse. Laura: Sì, forse è meglio che lasci. Aida: Vengo a casa e faccio fatica a trovarti dove ti ho lasciato. Hai idea di che cosa sto parlando? Avere a che fare con la gente. Render conto. Stare a sentire. Rispondere. Prestare attenzione. E risolvere I nodi, ammansire gli irati, abbindolare I potenti … È tutta una vita che ho a che fare con la gente. E io so come si fa. Hai sentito? So come si fa. Eccome se lo so. Avrei voluto insegnartelo, inserirti, farti guadagnare soldi… E tu invece “Non mi seccare, lo so com’è il tuo lavoro.”, mi rispondi. Sprezzante. Laura: Quella era mia madre. Io non ho detto niente. Tu non mi hai offerto di lavorare con te. Volevi farmi un regalo? Aida: Non voleva sapere. Niente. Non parlava. La ritrovavo dove l'avevo lasciata, e non parlava. Tu non vuoi sapere e io non riesco ad abituarmi. Mi sono sempre chiesta cosa potevo fare. Per lei. E da quando siamo rimaste noi due sole, per te. E anche adesso che devo tenere insieme quel che resta di noi, comincio con quella domanda - Cosa posso fare. Cosa posso fare. E non mi fermo lì. Provo a rispondere, faccio, mi inguaio. Sento il picchiare delle parole che dico, che ripeto, che lego l'una all'altra, le sento mentre mi escono dalle labbra ma soprattutto le sento dopo battere come su un metallo, come se dovessi ricordarle tutte, come se non ricordandole perdessi la strada. Ho cercato di spiegare a tua madre che si stava sprecando, che tutto quell’India non la portava da nessuna parte, che doveva smettere di buttar giù roba, roba, roba. Lei ingeriva troppo, a differenza di te. Con tua madre, l’inferno era sonoro. Oh, quante parole tornavano indietro, dopo le nostre liti, appena mi trovavo da sola. Intere conversazioni, non digerite, che si infilano qui sopra lo stomaco e poi salgono in testa. Le sento, le ripeto. Le sento adesso come allora, nel silenzio di questa casa in cui ho vissuto con mia figlia, la mia unica figlia. Non sono servite a niente. E allora si spezzano. Tornano come parole mal usate, scelte male forse. Odio questa eco che torna. Sono tutte voci che devono uscirsene, e non tornare. Sennò comincia il tormento, sennò sono cemento, e pesano. Bien que le français domine, le son de l'anglais n'est jamais loin, créant un « franglais » vivant qui reflète les influences anglaises historiques de la ville et ses connexions mondiales modernes. Ce mélange linguistique est plus que pratique ; c'est une déclaration culturelle, un mode de vie qui embrasse la diversité et favorise une identité communautaire unique. Vivre ici m'a appris bien plus qu'une nouvelle langue ; cela m'a offert un nouveau regard sur le monde, un regard qui valorise la diversité, célèbre le patrimoine et embrasse le changement. En marchant dans les rues animées, devant le mélange de sites historiques et de gratte-ciel élégants, je me rappelle que Montréal n'est pas seulement un endroit où vivre, mais une façon de vivre - qui façonne continuellement mon identité et la communauté qui m'entoure.

Como venezolana viviendo fuera de Venezuela, a menudo me encuentro diciendo la frase “Soy venezolana, pero llevo años viviendo aquí.” Frecuentemente, en respuesta me preguntan “¿Cómo está la situación en Venezuela?” A pesar de mi conocimiento sobre lo que está pasando en mi país, mi perspectiva como estudiante de psicología y escritora para la revista digital de salud mental “Inspire the Mind” me ha llevado a darme cuenta de que hay una brecha significativa en mi comprensión y la de los demás. La salud mental de Venezuela llega a primer nivel de atención. Sin embargo, es algo que a menudo queda eclipsado por las noticias y titulares. Las discusiones que yo suelo escuchar giran en torno de los aspectos económicos y políticos. Mientras que el estado de salud mental, y el impacto psicológico en individuos y comunidades está menos explorado, aunque sea muy importante. Esto no sólo se refleja en mis conversaciones, sino también en la información que está disponible en línea. Al buscar “Venezuela” en la web, te encuentras con una larga lista de artículos, videos y noticias, centrados en un análisis de la crisis con respecto a los aspectos económicos o políticos. Lo que no es tan fácil de encontrar es información detallada sobre otro aspecto, el estado de salud mental. Para los que no saben, desde la muerte de Hugo Chávez en el 2013, Venezuela se ha atravesado con una compleja crisis bajo el liderazgo de su sucesor Nicolas Maduro. Su elección como presidente, fue marcada por críticas de irregularidades electorales y desencadenó un periodo de represión y deterioro institucional que causó la negación de la democracia. La reducción de ayuda extranjera y la persistencia de políticas de gasto obsoletas han contribuido gravemente al déficit económico que provocó una escasez de dinero, comida, medicamentos y otras necesidades básicas. Como resultado, 4 millones de venezolanos necesitan ayuda humanitaria urgente, y más de 7.7 millones de personas han huido con la esperanza de encontrar mejores condiciones de vida en otro país. El deterioro de la salud mental está directamente relacionado con el deterioro de las condiciones de vida causado por la crisis humanitaria. La falta de esperanza, agravada por la pandemia de Covid-19, ha generado un incremento en los problemas de salud mental, especialmente entre aquellos que no pueden acceder a necesidades y servicios básicos. Los que pueden, se apoyan de familiares que viven afuera del país para mitigar la situación. Por ejemplo, en casa yo recuerdo escuchar conversaciones sobre maneras de enviar productos esenciales a familiares que aún viven en Venezuela. Sin embargo, para quienes no tienen esta opción, la situación es aún más difícil. La Federación de Psicólogos de Venezuela y la Sociedad Venezolana de Psiquiatría han informado que, durante la pandemia, los motivos principales de consulta se asocian con la depresión, ansiedad, los ataques de pánico, el estrés crónico, entre muchos más, los cuales van aumentando. Sin tratamiento adecuado, los que sufren de estas condiciones están en riesgo de suicidio, reflejado en el aumento preocupante de las tasas de suicidio entre 2018 y 2022, según Gustavo Páez del Observatorio Venezolano de Violencia. Algo que le añade otra capa de complejidad es la falta de transparencia al compartir información sobre la situación. Por ejemplo, el último informe sobre el número de suicidios fue publicado en el 2014, y muchas de las estadísticas disponibles pueden sufrir de subregistro. La falta de estadísticas actualizadas y fiables dificulta la comprensión precisa de la gravedad del problema. La crisis económica, política y humanitaria no solo impacta directamente en el bienestar psicológico de la población, sino que también dificulta el acceso a tratamientos adecuados, lo que continúa agravando la crisis en este ámbito de la salud. Según la doctora Antor, muchas personas en Venezuela actualmente tienen que decidir qué parte de la terapia pueden pagar, porque su sueldo no alcanza para comprar todo lo que está incluido en el tratamiento. Igualmente, si cuentan con el dinero necesario, se enfrentan con otros problemas. Por ejemplo, en 2019, ABC News escribió un artículo destacando la situación que se vive dentro del hospital psiquiátrico de Caracas, donde solo quedan 36 pacientes; camas oxidadas, cucarachas y otros insectos, basura por todas partes y falta de médicos y medicamentos. Según Wadarlberto Rodríguez, presidente de la Sociedad Venezolana de Psiquiatría, la disponibilidad de medicamentos como opción para tratar problemas psiquiátricos, como la depresión, ha disminuido un 70%. Antes, los psiquiatras venezolanos contaban con 20 tipos de antidepresivos. Ahora solo queda uno, el cual no está disponible regularmente e, incluso si lo está, las cantidades no son suficientes para administrarlos a los que tal vez se beneficiarían de ellos. Esta situación hace que muchas personas que pudiesen vivir vidas productivas y controladas, en términos de su salud mental, sufran gravemente. Es evidente que la crisis de salud mental en Venezuela es un ciclo perpetuo directamente influenciado por la situación del país. Con nuevas elecciones a la vista, el país está concentrado en ellas una vez más, con ojos de incertidumbre ante lo que puede ser un rayo de luz hacia una vida más estable o la continuación de una situación llena de retos y dificultades. Al terminar de escribir el artículo, me encontré reflexionando sobre la situación en Venezuela. Aunque yo sea orgullosa de ser venezolana, no puedo evitar sentirme un poco desconcertada por lo que está sucediendo en mi país. Al compartir este sentimiento con mi madre, ella me ofreció una perspectiva valiosa y diferente: “El venezolano es muy luchador y resiliente. Se ayudan unos a otros y se acompañan en las dificultades. Su carácter alegre y bochinchero a veces son las únicas herramientas con las que cuentan para mitigar la crisis.” Estas palabras me hicieron reflexionar sobre la importancia de no solo enfocarse en los aspectos negativos que han llevado al país a esta situación, sino también compartir la fortaleza que los venezolanos llevan consigo cada día, eso es lo que causa que yo sea orgullosa de ser venezolana.
- Salute, Ambiente e Diseguaglianze
L'Organizzazione Mondiale della Sanità (OMS) definisce la salute non come assenza di malattia ma come la presenza di benessere fisico, sociale e mentale. Salute è buona qualità della vita, unione di benessere psicofisico e relazionale, considerando anche le sue componenti sociali ed economiche. Io sono una psichiatra che lavora a Cagliari nel Servizio pubblico, mi occupo di pazienti e famiglie che mostrano sofferenza psichica. La salute mentale occupa un posto importante nella definizione di salute perché si dice anche, sempre secondo una definizione dell'OMS, che non esiste salute senza salute mentale. La correlazione tra istruzione, situazione socioeconomica e ambiente è diventata per me nel tempo sempre più importante e negli ultimi mesi sono diventata ancora più sensibile a questi temi dal momento in cui ho iniziato il mio impegno politico nella mia regione e nella mia città. Ho scritto questo pezzo per dare forma al mio pensiero politico e per evidenziare quanto si possa e si debba fare anche a livello locale. L'accesso all'istruzione e la possibilità di avere un lavoro che permetta una vita dignitosa sono i punti di partenza che devono avere tutti i cittadini per potere essere integrati nella società e avere possibilità di scelta (“pieno sviluppo della persona umana” così come definito dalla Costituzione Italiana). È dimostrato che la poca istruzione e le condizioni socio-economiche incidono su speranza di vita, malattia e mortalità. Vivere in una situazione di scarsità economica o essere poco istruiti porta inevitabilmente a non avere la possibilità di accedere alle cure. Da una parte, il Sistema Sanitario Nazionale italiano, a causa del sottofinanziamento ma non solo, non riesce a garantire a tutti le cure, quindi chi può permetterselo economicamente paga e chi non ha disponibilità e non può pagare non si cura; dall'altra, la poca istruzione non permette di avere gli strumenti necessari per poter avere cura di se. Il titolo di studio è fortemente collegato ad alcune patologie: per esempio, l'ipertensione, le malattie cardiovascolari, l'obesità sono più frequenti nelle persone poco istruite. Alcuni esempi: l'ipertensione colpisce il 43% delle persone che ha solo la licenza elementare e la percentuale scende andando avanti con gli anni di studio. È poi ipertesa una persona su 4 tra chi dichiara di avere molte difficoltà economiche (contro 1 su 6 di chi non ha problemi economici), l'obesità è presente nel 14,5% delle persone con basso titolo di studio e nel 6% in quelle più istruite, rinuncia alle cure per motivi economici il 69% delle persone poco istruite e il 34% dei laureati. Anche l'accesso agli screening oncologici è ridotto nelle persone poco istruite. Allo stesso modo, contare su un unico stipendio molto basso può rivelarsi letale quanto fumare o avere il diabete. E questo perché non ci permette di scegliere davvero il nostro stile di vita. Anche in Italia, un quarto delle morti da tumori è associato a bassa istruzione. Altro punto fondamentale riguarda il rapporto tra Salute e ambiente. Partiamo dal concetto che senza un ambiente sano è difficile essere in salute, ossia la salute è possibile solo con un cambiamento dell'ambiente in cui si vive. E quando parliamo di ambiente intendiamo nel senso del clima ma anche dell'ambiente urbano che riguarda le città, del verde e dell'urbanizzazione. E anche degli stili di vita. Il modello “One Health” considera necessario l'approccio integrato e unificante per bilanciare e ottimizzare la salute delle persone, degli animali e dell'ambiente, ossia la salute degli esseri umani, degli animali e degli ecosistemi è strettamente interconnessa. È un modello non recente ma che ultimamente ha ripreso spazio ed è stato riproposto con sempre maggiore forza anche dall'ufficio europeo della OMS. Se ci inoltriamo nel discorso legato alle città e all'urbanizzazione, ricordiamo che sono sempre in aumento gli spostamenti della popolazione verso le città, con tutti i conseguenti problemi, può essere importante esaminare almeno brevemente alcuni concetti. Uno di questi è quello della rigenerazione urbana. La rigenerazione urbana si occupa di promuovere il miglioramento dell'ambiente urbano attraverso la gestione integrata degli aspetti fisici e degli aspetti sociali e “sostenendo l'attivazione dell'intelligenza collettiva”. Riguarda per esempio la riqualificazione, riuso, e recupero del patrimonio edilizio, arrivando alla rigenerazione di quartieri e di parti della città per arrivare a gestire i cambiamenti climatici attraverso azioni concrete di adattamento e miglioramento dei sistemi insediativi. Quindi è rivolta alla tutela del suolo, ma non solo, anche alla tutela del tessuto sociale all'interno delle città. Per fare questo è necessario integrare le conoscenze urbanistiche (il verde, la presenza di fabbriche, la mobilità) con quelle sanitarie (situazione demografica e epidemiologica) e con quelle sociali (sicurezza, presenza di associazioni di quartiere, volontariato), svolgendo quindi anche un'azione che riguarda la mappatura delle condizioni di salute e di benessere delle zone della città Nell'agenda ONU 2030 per lo Sviluppo Sostenibile vengono stabiliti 17 obiettivi da mettere in atto per riuscire a raggiungere un miglioramento globale del pianeta attraverso la lotta alla povertà, la produzione di azioni di pace e prosperità e attraverso azioni che proteggano il pianeta e il suo ambiente. Nell'obiettivo 11 si parla di “rendere gli insediamenti urbani inclusivi, sicuri, resilienti e sostenibili”, promuovendo quindi azioni urbane che riducano i rischi, compresi quelli climatici e delle emissioni dei gas serra. Queste azioni devono essere svolte sia con interventi a livello nazionale e regionale sia con azioni a livello locale. I Comuni infatti hanno la possibilità di intervenire su spazi urbani come piazze, parcheggi, quartieri degradati, comparti dismessi, mettendo in atto azioni di contrasto all'inquinamento e al cambiamento climatico, per esempio introducendo dove manca il verde pubblico, creando strade alberate e ombreggiate che riducano il calore e migliorino la qualità dell'aria, agendo con interventi che migliorano la gestione delle acque (per esempio con i giardini della pioggia e i parchi urbani inondabili) sino alla realizzazione dei più complessi eco-quartieri. Queste soluzioni basate sulla natura hanno come scopo quello di mantenere e restituire permeabilità ai suoli urbani, di introdurre la vegetazione che a sua volta agisce contemporaneamente sulla gestione sostenibile delle acque, sul miglioramento del microclima e sul contrasto al calore. I comuni hanno anche la possibilità di intervenire sull'inquinamento atmosferico agendo sulla mobilità (strade, parcheggi, piste ciclabili, trasporto pubblico, car e bike sharing) e sull'inquinamento acustico gestendo gli spazi pubblici. Gli spazi pubblici devono essere intesi come gli spazi della socialità, dell'integrazione e dell'inclusione che devono quindi essere garantiti come patrimonio di tutti. E le persone riescono a stare bene nei luoghi pubblici se si sentono al sicuro, se si trovano in una condizione di comfort e benessere psicofisico, se percepiscono una buona qualità della vita. Il perseguimento di tale obiettivo, che può apparire ad un primo sguardo ambizioso, deve comunque passare attraverso la tutela di alcune garanzie quali il diritto al verde, al benessere, alla salute, alla accessibilità e alla mobilità attiva. È necessario quindi includere la salute nello sviluppo urbano perché è un fattore di crescita e coesione nelle città e tra le persone ma è anche un fattore di inclusione sociale. Come ultimo punto vorrei analizzare il rapporto tra Salute e diseguaglianze. Se è vero che cittadini meno istruiti e poveri hanno maggiori difficoltà ad accedere non solo ai percorsi sanitari ma anche ai percorsi di prevenzione e promozione della salute, è necessario anche lavorare affinché le diseguaglianze si riducano. E quando si parla di diseguaglianze si intende anche porre attenzione ad altri gruppi di cittadini quali le persone con problemi di salute mentale, le donne, i migranti, le persone LGBT, le minoranze, gli anziani, quelle che vivono in una situazione di solitudine e di isolamento sociale, i bambini e i giovani. Ricordiamo che godere del più̀ alto standard di salute raggiungibile è uno dei diritti fondamentali di ogni essere umano. Allo stesso tempo è importante procedere per far sì che si assicuri l'equità sanitaria perché anche percorrendo questa strada si può produrre prevenzione e salute. Per concludere: è fondamentale ragionare immaginando una rete che collega istruzione, lavoro, ambiente e salute, immaginandoli come punti interconnessi tra di loro e in totale relazione. E avendo in mente che tale relazione può essere disfunzionale e quindi patogena e generatrice di malattia (nel senso più ampio del termine) oppure sana e funzionale e quindi generatrice di salute. Pertanto, si deve dare priorità alle politiche integrate che includano non solo la sanità ma anche l'occupazione, l'istruzione, la digitalizzazione, la protezione sociale, la sicurezza finanziaria, la pianificazione urbana, l'accesso alla cultura e ai media, l'ambiente e il clima.
